摘要

台灣於1994年65歲以上的老年人口達7%，人口的老化已達世界衛生組織所定義的「老化國家」之列。依據推估將在2021年達雙倍成長至14%，因此，人口老化快速增加將帶來長期照護需求快速成長。近年來，我國長期照護體系之建立，在政策上逐漸強調社區化長期照護的發展，以支持老人「原居養老」為導向，希望能以可負擔的居家和社區式服務協助身心功能障礙者。未來家庭照顧者的社區照顧支持網路的建構，在政策與實務上，應以有效結合正式支持系統，維持與加強現有非正式支持系統。專業護理在現今發展已開始朝向夥伴關係的形式以符合未來健康照護系統之需求。長期照護服務更需整合健康照護團隊來提供服務，團隊的專業人員共同合作為其首要條件。同時，社區護理人員及護理學生教育應加強團隊合作的訓練。社區護理人員需配合角色的發展而成長，以符合公共健康議程。

Taiwan's population has reached the standard of ”aging society” defined by the World Health Organization (W.H.O.). The population of aged 65 and over was 7% since 1994. The aging growth will be double to 14% in 2021. With this rapidly aging tendency, the need for long-term care services will increase dramatically. The government has launched the long-term care system for many years in Taiwan. The policy of ”aging in place” aims to emphasize on making home and community-based services affordable to the disabled. In order to provide community care network to family caregivers in the future, there should be a link up between both formal and informal services, politically and practically. Professional nursing has already begun forming in partnership for shaping the health care system of tomorrow. Interdisciplinary collaboration is essential to the team’s long- term care. The development of increased community health nurse and nursing student skills related to interdisciplinary team work and program development. The scope for developing the role of community health nurses in relation to meeting the public health agenda.
