冠狀動脈鈣化評分之臨床應用與分析

Clinical Application and Analysis of Coronary Calcium Scoring

摘要


鈣化為冠狀動脈粥狀硬化病程中的伴隨產物,鈣化的斑塊量和冠狀動脈疾病有一定的相關性。因此藉由量化冠狀動脈的鈣化沉積,可以對罹患冠狀動脈疾病的風險作分級評估,進而達到早期篩檢、預防以及病程評估、藥物控制療效評估之目的。本院在民國93年3月16日至7月31日為止,共針對369位無症狀的受檢者進行鈣化評分的掃描。將所得到的結果依據性別、年齡以及鈣化的分佈位置作分析,結果顯示男性在55歲女性在65歲之後即進入冠狀動脈快速鈣化沉積期,相對的罹患冠狀動脈疾病的風險就較高,而鈣化風險最高的為冠狀動脈左前降枝。本研究中以多列偵檢器電腦斷層鈣化評分檢查法來評估冠狀動脈鈣化,我們認為此方法為一安全、快速且精準的檢查方法之一。

Coronary calcium is a product of systemic atherosclerosis, which is responsible for coronary artery disease. Thus, the technique of measurement of coronary calcified plaque volume will be a valuable tool in clinical research and patient care. Moreover, it provides a marker with which to guide the initiation of prevention, drug therapy, clinical course and clinical outcome. From 16(superscript th), March 2004 to 31(superscript st), July 2004, 369 initially asymptomatic individuals who underwent MDCT for calcium screening. These data, which were analyzed base on age, gender and distribution. We obtain that men will have the high risk of CAD after 55 years old and women after 65 years old depend on rapid deposition of calcium in the coronary artery. Calcified plaque found in the anterior descending branches of left coronary artery will cause much higher risk also. Multi-detector row computed tomography (CT) for coronary calcium scoring is a safe, rapid, and accurate method of coronary calcium quantification.
