待產過程中,藉由陰道指診法得知子宮頸擴張做為產程進展與待產時機的評估。評估結果不正確將造成產程預估錯誤甚至影響安全。本研究專案目的為提升新進人員陰道內診評估正確率,進而改善臨床照護品質。本回顧性研究分析台灣某區域教學醫院之未曾接受產房訓練新進護理人員。期間為2023年6月1日至2023年10月31日,以可信賴專業活動(EPA)訓練新進護理人員執行陰道內診,製作臨床指引和擬真教具教材、規劃教學評量及教學回饋。具體成效的評估定義為陰道內診EPAs任務完全達成Level 4為100%計算。期間共6位新進護理人員,陰道內診正確率的知識與技能項目提升至100%,執行陰道內診EPA可信賴程度在半年訓練期內全數達到Level 4。陰道內診能力提升達成專案目的。透過本專案改善措施,建置陰道內診EPA創新教學模式,有效提升新進人員對陰道內診評估識能,正確執行任務並提升產婦之照護安全。
Pelvic examination is used to evaluate cervix dilatation for assessing labor progress and timing of delivery. Incorrect assessment results will lead to labor prediction errors and even affect patient safety. This research program aimed to increase the correct rate of pelvic examination among novice nurses. The retrospective study investigated novice nurses in a regional hospital in Taiwan who had never trained for delivery. The subsequent research period was from June 1, 2023, to October 31, 2023. Entrust-able Professional Activities (EPAs) teaching, clinical guidelines, and immersive teaching aids were prepared to train novice nurses to conduct pelvic examinations, followed by teaching assessments and feedback. Target outcome was defined as the percentage of nurses achieving 100% correct rate. Six nursing staff were recruited during the study period. All of them achieved a 100% correct rate and level 4 EPA reliability within the six-month training period, and the current program goal was reached. The improvement measures of a novel EPA teaching model for pelvic examinations of the current program can effectively increase the competence of the novice in vaginal examinations, allowing a correct duty and elevating the care safety of the parturient women.
