摘要

鼻及鼻竇炎是耳鼻喉科門診常見的疾病，醫師往往根據病患主訴及理學檢查的發現給予診斷及治療。一19歲女性，因左側流鼻水、打噴嚏、喉嚨癢、咳嗽及左側前額頭痛1個多月，曾於其他診所就診，診斷均為亞急性鼻及鼻竇炎，但是，治療一直是沒有效果；本診所發現其實是該側鼻腔內水蛭寄生，於局部麻醉後，在鼻內視鏡下以鼻鑷取出。特報告此案例，除了罕見外，只要醫師肯多花點時間診視鼻腔內部，並審視是否為靜態的病灶，其實並不難誤診。

Rhinosinusitis is a common disease in otorhinolaryngologic clinics. It is diagnosed and treated by chief complaints and physical examinations. A 19-year-old female patient presented with left rhinorrhea, sneezing, itchy throat, cough and left frontal headache for over 1 month. Unilateral subacute rhinosinusitis was impressed, and medicine failed to cure her symptoms at other clinics. However, unilateral nasal leech infestation was identified at our clinic. After the nasal cavity was locally anesthetized, the worm was removed under endoscopy. Herein the rare case is reported as a warning that we should take more time to evaluate a nasal lesion, and a nasal leech infestation will not be misdiagnosed as unilateral rhinosinusitis.
