一位前列腺癌末期個案因腫瘤壓迫導致下肢癱瘓的護理經驗

摘要

本文旨在探討一位54歲前列腺癌末個案因腫瘤侵犯骨骼壓迫神經導致下肢癱瘓，所面臨的身心衝擊與調適之過程。照護期間為2008年8月30日至2008年9月26日，藉由直接護理、觀察、會談等方式收集資料，運用Gordon 11項健康功能評估，發現個案有疼痛、身體活動功能障礙、無望感、身體心像紊亂、腹脹等護理問題。筆者於照護期間運用同理心提供整體性的護理措施、與適切的護理指導，協助個案對疾病進展的心理調適，減輕身體疼痛，接受身體心像的改變，幫助個案重新面對人生、滿足生理、心理、靈性及社會層面的需求。

This paper aims to explore the physiopsychological shocks and adjustment process of a 54-year-old patient with terminal-staged prostate cancer leading to paraplegia by tumor compression. The caring period was from August 30, 2007 to September 26, 2007. We collected information by direct care, observation and interview. By using functional evaluation of the healthful Gordon 11, we found patient had pain, physical activity dysfunction, fear, hopelessness, body image disturbance, abdominal distension and other nursing problems. During the application of holistic nursing interventions, we use empathy on the nursing care to provide appropriate guidance for psychological adjustment for disease progression in order to alleviate pain and help case to accept body image change, face real life and meet the physiopsychological, spiritual and social dimensions of the needs.
