住院病人接受外籍監護工照護之滿意度探討

Hospitalized Patients Satisfaction of Care Received from Foreign Care Aides

摘要


隨著人口老化,罹患慢性病及功能的衰退,使得需要照顧老年人數日益增加,政府考量安定社會經濟發展,引進外籍監護工,成了長期照顧體系的重要角色。本研究之主要目的爲:(1)瞭解外籍監護工照護壓力。(2)瞭解病人對外籍監護工照護之滿意度。(3)比較病人人口學特質、外籍監護工人口學特質、照護壓力與病人對外籍監護工照護滿意度之差異。研究以中部某區域教學醫院外籍監護工及被照護之病人爲研究對象,進行橫斷面調查,收得108名有效樣本進行分析。以有效自擬式問卷(CVI值介於75%-100%,Cronbach'α=0.91)進行資料收集。結果顯示:(1)外籍監護工壓力平均分數爲1.51,介於沒有壓力與稍有壓力之間。照護壓力大小依序爲「常被罵」、「在醫院工作無法自己決定」、「協助鼻胃管管路照護」。(2)病人對外籍監護工之滿意度平均分數爲3.67,表中上程度的滿意。(3)病人人口學特質與外籍監護工人口學特質在病人滿意度上皆無顯著性差異。另外照護壓力與病人滿意度呈正相關。本研究結果未來可提供醫院、雇主改善外籍監護工照護壓力以及提升住院病患照護品質之參考。

Along with aging in population, more elderly suffers from chronic illness and their function declining which make them need care. Considering of stabilizing the social-economic development, Taiwan's government enact of a foreign labor policy has allowed foreign care aides becoming an important roles in long term care system. The purposes of this research were to: (1) investigate caring stress perceived by foreign care aides; (2) explore patients' satisfaction regarding care received from foreign care aides; and (3) compare the differences of patients' demographic characteristics, foreign care aides' demographic characteristics, and perceived caring stress on patients' satisfaction of care. A cross-sectional survey was conducted and 108 subjects were recruited from a regional teaching hospital in the middle of Taiwan. A valid self-developed questionnaire (CVI=75%-100%, Cronbach's α=.91) was used to collect data. The results showed: (1) the mean score of care stress was 1.51, means no or little care stress of foreign care aides, and they perceived ”be blamed of”, ”unable to decided work content by herself”, and ”assisted N-G feeding care”, respectively. (2) The mean score of patients' satisfaction with care was 3.76, means moderate or more satisfaction of foreign care aides' services. (3) There were no significant differences in patients' satisfaction between patients’ demographic data, foreign care aides' demographic data. Furthermore, the results revealed a positive correlation between care stress and patients' satisfaction. Findings of this study provide reference for government, hospital managers, and employers in developing strategies to decrease foreign care aides' care stress and improve care quality in hospitals.
