The Correlation between Children's Disruptive Behaviors and Specific Dental Procedures

Purpose: The purpose of this study was to study the correlation between children's disruptive behavior and specific dental procedures. Methods: One hundred and twelve children, aged 15 to 111 months, were included in the present study from June 2000 to May 2002. The selected patients for this study were treated by the same pediatric dentist in a dental clinic in Taipei. The patient's behaviors in the operatory were videotaped during the whole dental treatment. We classified the whole dental treatment into sixteen specific dental procedures and rated children's behaviors by four raters using modified Wus' Behavior Rating System. Eight hundred and sixty appointments were recorded and analyzed. Statistical evaluation was performed by independent-samples t-test and chi-square tests. Results: The specific dental procedures which children's disruptive behaviors occurred more frequently were ”Local anesthesia injection” (17.0%), ”Introduction of high-speed handpiece” (15.6%), ”Introduction of low-speed handpiece” (14.8%), and ”Rubber dam application” (14.3%). The proportions of children's disruptive behavior were higher in the Tell-Show-Do (TSD) appointments than in treatment appointments when using the same dental tools. There was significant difference between TSD appointments and treatment appointments when using low-speed handpiece (p<0.05) and suction (p<0.01). The disruptive behaviors in the following procedures ”See the dentist” (p<0.01), ”Hop into the chair” (p<0.01), ”Introduction of high-speed handpiece” (p<0.05), ”Using of high-speed handpiece” (p<0.01), and ”Stainless steel crown fabrication” (p<0.05) were significant difference between younger (<36 months of birth) and older children (≧36 months of birth). Conclusions: The proportion of the disruptive behaviors occurred 18.0% in total specific dental procedures. The procedures which children's disruptive behaviors occurred frequently were as follows: ”Local anesthesia injection”, ”Introduction of high-speed handpiece”, ”Introduction of low-speed handpiece”, and ”Rubber dam application”. Children's disruptive behaviors were reduced after the TSD appointments.
