摘要

深層顱骨穿刺傷為嚴重的腦傷，不當或延遲的治療可能引起不良預後，我們報告一位86歲男性，遭螺絲起子插入額頭，病人在住院第7日死亡，文中討論這類病人的肇因、檢查、處置、注意事項、併發症與預後，深層腦部穿刺傷經影像檢查與血管攝影評估後，若無大血管損傷引發出血的可能，可移除異物並施作清創術與抗生素治療。

Skull deeply penetrating injury was a kind of severe brain injury. Improper or delayed treatment may cause poor prognosis. We reported an 86 years old man suffered from screwdriver insertion on the forehead. This patient died the seventh day later. We reviewed the literature and discussed the causes, examination, management, legal aspect, complication and prognosis in this kind of injury.
