提升血液透析病人血色素之改善專案
Project of Improving Anemia Management in Hemodialysis Patients
摘要
背景：統計2019年6-9月本血液透析中心病人血色素值平均為9.73 g/dL，≧8 g/dL的比率為87.90%，未達腎臟醫學會標準之血色素平均值≧10 g/dL，≧8 g/dL的比率≧90.00%，故引發改善動機。目的：提升血液透析病人之血色素值。解決方法：改善單位作業流程、護理師在職教育、製作高鐵高蛋白飲食及貧血衛教單張及影片、病人衛教，提升護理師在認知、技術、情意方面給予病人的幫助。結果：平均提升為10.12 g/dL，≧8 g/dL的比率提升至91.60%，達成目標值，成功符合評量標準，提升透析品質。結論：由本專案的改善過程，提供完整貧血治療流程規範，做為臨床上參考檢視的方針，有效提升血液透析病人之血色素。

Background: From June 2019 to September 2019, the average hemoglobin level of patients in our hemodialysis center was 9.73 g/dL, and the ratio of hemoglobin ≥ 8 g/dL was 87.9%. Both the average hemoglobin ≥ 10 g/dL and the ratio of hemoglobin ≥ 8 g/d did not reach the target value (average hemoglobin value more than 10 g/dL and the ratio hemoglobin of ≥ 8 g/dL more than 90%) from the Taiwan Society of Nephrology, so we would like to improve it. Objective: To increase the hemoglobin level of hemodialysis patients. Method: There are 3 methods to help our patients by enhancing our members of cognitive, affective and psychomotor: 1. Improve standard operating procedure 2. In-service education for nurses 3. Edit health education leaflets and videos about high-iron and high protein diet, and anemia Results: The average of hemoglobulin is elevated to 10.12 g/dL, and the ratio of hemoglobulin ≥ 8 g/dL is also increased to 91.6%, which reached the target value, and it achieved the target with improving the quality of dialysis. Conclusion: According to the process of our project, we provided the complete treatment protocol as a guideline, which can effectively improve the hemoglobulin level of hemodialysis patients.
