正念課程介入對高危險妊娠安胎孕婦憂鬱、焦慮之成效探討

The Efficacy of a Mindfulness-Based Intervention for Depression and Anxiety of Women Undergoing Tocolysis

摘要


背景:高危險妊娠安胎孕婦面臨早產高風險與負面心理之健康問題。有效的介入是醫療人員須面對之相當重要的課題。目的:本研究旨在探討正念課程對於高危險妊娠安胎孕婦憂鬱與焦慮的介入成效。方法:採準實驗研究設計,以婦產科病房的高危險妊娠安胎孕婦為對象,實驗組(n = 14)接受常規護理指導及為期4週、每次30分鐘之正念課程;對照組(n = 16)則接受常規護理指導,在介入前測、第四週後測及第六週追蹤以問卷收集資料,工具包含人口學、愛丁堡產後憂鬱量表、情境-特質焦慮量表,以廣義估計模式進行分析。結果:關於憂鬱方面,實驗組相較於對照組在後測(p = .025)及追蹤(p = .022)分數皆顯著降低;關於情境-特質焦慮於實驗組隨時間下降,但與對照組兩組間後測及追蹤分數均未達到統計顯著差異(p > .05)。結論:以正念課程介入,有助於高危險妊娠安胎孕婦降低憂鬱,未來可整合正念課程到高危險妊娠安胎孕婦住院照護的社會心理支持模式建構之參考。

Background: High-risk pregnancies with tocolysis pose increased risks of preterm labor and psychological challenges. Implementing effective interventions during the perinatal period is crucial for healthcare providers. Purpose: This study aims to evaluate the effectiveness of a mindfulness intervention program in reducing depression and anxiety symptoms among women with high-risk pregnancies undergoing tocolysis. Methods: The study, using a quasi-experimental design, included women in obstetrics and gynecology wards. The experimental group (n =14) received nursing instructions and weekly 30-minute mindfulness sessions for four weeks, while the control group (n=16) received only regular nursing instructions. Data collection at pre-intervention, post-test (week four), and follow-up (week six) utilized the Edinburgh Postnatal Depression Scale and State-Trait Anxiety Inventory. Analysis employed the generalized estimating equations method. Results: The experimental group showed a significant decrease in depression scores at post-test (p =.025) and follow-up test (p =.022) compared to the control group. However, there were no significant differences in state-trait anxiety, between experimental group and control group at posttest and follow-up test (p>.05). Conclusions: The study indicates that incorporating the mindfulness intervention into inpatient care for high-risk pregnancies with tocolysis effectively reduces depression, enhancing a comprehensive psychosocial support model.
