The Attributable Factors of Hyperuricemia in Female Patients with Chronic Kidney Disease Stratified by Status of Diabetes

Background. Hyperuricemia is regarded as a risk factor for cardiovascular disease, chronic kidney disease (CKD) and death in subjects with type 2 diabetes. This study investigated the association factors of hyperuricemia in female subjects with CKD. Methods. 34,372 persons attended the national health insurance health check-up program in our hospital from 2003 to 2009, which included laboratory tests, medical history, smoking, alcohol, and betel nut chewing habits. We analyzed differences in demographics, clinical characteristics, and comorbidities in female CKD patients stratified by diabetes and hyperuricemia statuses. We used a multivariate logistic regression model to investigate association factors with hyperuricemia. Results. 2,354 female participants had CKD. The risk of hyperuricemia increased significantly in subjects who were overweight or had hypertension or low eGFR in both diabetes and non-diabetes groups. The multivariate logistic regression analysis revealed that older age [adjusted odds ratios (aOR): 1.807, 95% confidence interval (CI): 1.053-1.763], BMI ≧ 25 ( aOR: 1.807, 95% CI: 1.488-3.195), hypertension (aOR: 1.349, 95% CI: 1.098-1.657), and lower eGFR (30-44, aOR: 2.378, 95% CI: 1.732-3.266; <30, aOR: 5.412, 95% CI: 2.930-9.996) were associated with increased risk of hyperuricemia in the non-diabetes group. In the diabetes group, the risk factors for hyperuricemia were BMI ≧ 25 (aOR: 1.713, 95% CI: 1.149-2.553) and lower eGFR (30-44, aOR: 2.411, 95% CI: 1.429-4.066; <30, aOR: 3.897, 95% CI: 1.662-9.139). Conclusion. We found positive correlations of hyperuricemia with BMI and low eGFR in both non-diabetes and diabetes groups, and negative correlations of hyperuricemia with elderly age and hypertension in the diabetes group, but positive correlations in the non-diabetes group.
