晚期腎臟疾病合併多重共病之高齡病人疼痛評估與處理－個案分享
Pain Management in An Elderly Woman with Advance Chronic Kidney Disease and Multiple Comorbidities
摘要
在台灣，晚期腎臟疾病（advanced chronic kidney disease）患者的人數伴隨高齡社會（aged society）漸漸增加，雖然新發生透析人數仍每年增加，但隨病人自主權利法（Patient Right to Autonomy Act）之通過，預期因多重共病而選擇不進行透析治療的高齡病人，也將與日俱增。因此如何適時提供緩和醫療介入，積極且安全的給予症狀處理，便顯得格外重要。本篇以一位老年晚期腎臟疾病患者，因骨折與使用引流管引起的疼痛為例，希望能提供晚期腎臟病患，尤其是選擇不進行透析的病人，相對安全有效的疼痛用藥選擇。

Ongoing growth of the geriatric population has brought up the number of patients suffering from advanced chronic kidney disease (CKD) and entering dialysis each year. On the other hand, with the Patient Right to Autonomy Act implemented in 2019, the number of geriatric patients with multiple comorbidities who refuse renal replacement therapy and choose conservative kidney management is expected to rise. Therefore, management of symptoms, such as pain, itchy skin, and weakness, with both adequate and safe measures become an increasingly important issue. We reported an elderly woman diagnosed with advanced CKD and experiencing persistent pain caused by fracture and pigtail insertion. We hoped to provide insights into effective use of analgesics with for advanced CKD patients, especially those without dialysis, with minimum adverse effects.
