Oral melanotic macule is a benign pigmented lesion that often raises esthetic concerns, particularly when occurring on visible areas such as the lips. While various treatment options exist, cryotherapy offers a minimally invasive approach. This case report presents a 42-year-old female with two melanotic macules on her lower lip measuring up to 5 × 3 mm. The patient was treated with cryogun cryotherapy, with four treatment sessions performed at two-week intervals. Progressive depigmentation was observed after each treatment session, and complete regression of the lesions was achieved after four treatments, with no scarring or complications. The healing process was uneventful, with minimal discomfort reported. Follow-up at six months showed sustained results with no recurrence. This case demonstrates that cryotherapy is an effective, safe, and minimally invasive treatment option for oral melanotic macules, with advantages including procedural simplicity, minimal discomfort, and favorable esthetic outcomes.
