摘要

發展遲緩幼童的餵食問題除了可因器質性(organic)病變造成外，也可能是心理方面的障礙所形成的。本篇所報告的病例就是一位發展遲緩且在餵食時經常嘔吐的幼童，她並沒有口部器質性的障礙，乃因心理因素造成其餵問題。經由心理方面著手治療後，其餵食情況已接近正常，可以自行進食固體食物，且其語言的發展也因而進步不少，故提出報告，以爲參考，

The causes of feeding problem in psychomotor retarded children include both organic and psychological aspects. The treatment course of such a child who vomited frequently during mealtime is reported. This 2 years old girl had reached nearly normal development in her gross and fine motor function after 11 months' neurodevelopmental facilitation. But her feeding problems persisted since 2 months old, and she also had markedly delayed speech development. So she was refered to us for her feeding problems. After detailed examinations and observations, no specific organic cause was found for her feeding problem. Strog anxiety as her mother and some hyperactive and aggressive behaviors were noted in this case too. So we start the treatment program through psychological approach. We insisted that mother should follow the same rules as us in the followings: (1) Never react too vigorously when the girl vomited again. Just treat it calmly. (2) Don't give snacks to the girl between meal time if she refused regular meal. (3) Try different kinds of food each at a time. Change the character of food from liquid to semisolid then solid gradually. (4) Encourage the girl to eat by herself. Let her play with spoon, bowl and cup. Make the atmosphere of feeding cheerful and interesting.
At the last follow up (when the girl was 3 years and 4 month old), she could eat table food with spoon independently without vomiting, and use a few simple sentences for communication. Those aggressive and hyperactive behaviors improved much, too. We suggest that psychological causes should also be considered in the evaluation and management of feeding problems in psychomotor retarded children.
