青春期學童身高、體重、BMI值與初經發生年齡相關性之研究

摘要

本研究調查台北與基隆兩地區，國小五、六年級和國中一、二、三年級共1942位年齡在10-15歲的學童，探討兩個不同地區的學童在快速成長期間，初經發生率與身高、體重、BMI值可能的關係。兩地學童的平均身高、平均體重並無顯著差異。分析兩地區學童生長速率可知兩地區學童身高快速增加是在10-13歲間，13歲之後增高速率趨緩。台北學童體重快速增加是在11-13歲間；基隆學童體重快速增加是在10-12歲間。台北地區學童初經年齡中位數是11.92±0.85歲，基隆是11.90±0.90歲。同一年齡層的學童無論身高、體重、BMI值，有初經者均與未有初經者有顯著差異，有初經者BMI值均大於18，無初經者BMI值則小於18。綜合兩地區學童之初經發生率，在10-11歲為9.68%，11-12歲為21.08%，12-13歲為71.85%，13-14歲為90.54%，14-15歲為97.82%。約有50%的學童在12-13歲期間進入性成熟。全部調查資料中，初經發生最早是8.01歲。比較肥胖與非肥胖者初經發生的年齡，同一年齡層，肥胖者比非肥胖者，初經發生率高，尤其在11-12歲快速成長期，二者間的差異性最大。經卡方檢定，11歲以上的學童肥胖與初經發生年齡有顯著關聯。

The investigation was conducted for 1942 adolescent students of grade 5-9, age 10-15 in Taipei and Kelung area. We tried to understand height, weight, body mass index (BMI) and their relationship to menarche age while the students were at their rapid growth rate. Both in Taipei and Kelung areas, student height increased sharply at age 10-13, growth rate slow down after 13 years old. Body weight increased dramatically at age 11-13 in Taipei, and similar results was found at age 10-12 in Kelung area. No significant difference was found in terms of average height, weight between the Taipei city and Kelung city. The median age at menarche was 11.92±0.85 in Taipei and 11.90±0.90 in Kelung. Comparing the height, weight, and BMI value, same age students who reached menarche were significantly higher than premenarcheal ones. The 8MI were higher than 18 for students reached menarche, while the BMI of premenarcheal students were less than 18. The frequency of menarche for age 10-11 students was 9.68%, for age 11-12 was 21.08%, for age 12-13 was 71.85%, for age 13-14 was 90.54%, for age 14-15 was 97.82%. Fifty percent of age 12-13 students already experienced their menarche. Among the students investigated the earliest menarche happened at age 8.01. Comparing the same age students, the frequency of menarche was higher for obesity than those of non-obesity。The difference was significant at age 11-12, while the students were their rapid growth rate. Based on Chi square test, students above 11 years old, a significant association exists between menarche age and obesity.
