醫療資訊應用趨勢-南部某醫學中心e ICU電子白板推行之經驗分享

Trends of Medical Information Technology: Electronic Intensive Care Unit Whiteboard Implementation Experience in a Medical Center in Southern

摘要


目的:由於科技發展,各種相關資訊也陸續導入醫療環境藉以簡化工作流程,此研究分析臨床使用者需求,導入e ICU電子白板概念,運用資訊安全把關,規劃人形圖設計、管路及各項警示,進而分享推行臨床電子需求畫面之經驗。方法:透過e ICU白板建置,將病人評估之核心概念連結至加護病房臨床資訊系統,以圖示方式呈現病人身上管路註記及警示符號,建構e ICU電子白板全貌。結果:此e ICU電子白板研究為導入臨床資訊系統、統一介面設計及圖示檢視。加護病房氣管內管自拔發生率從2014年0.11%降至2016年0.0015%之成效,其他使用後正向回饋包括:節省時間佔37.11%,即時查詢,避免遺漏佔29.56%、資訊連結清楚易懂佔21.38%等,期待將此推行建置電子資訊系統之過程及經驗,提供各健康照護機構醫療資訊應用之參考。結論:透過e ICU電子白板的設計,可即時連結及取得病人相關醫療照護訊息,其可近性及便利性可作為提升臨床照護效益策略之一。

Purpose: Because of developments in science and technology, relevant information from various departments have been gradually integrated into one platform in the medical environment to simplify the workflow. This study analyzed the needs of clinicians, implementation of electronic whiteboards to create electronic intensive care units (eICUs), use of information security checks for human figure design, and patients' pipelines and warning signs. The objective was to share an experience of implementing an electronic whiteboard in the clinical setting. Methods: Through eICU whiteboard settings, the core concept of patient assessment was linked to an intensive care clinical information system. This paper presents an overview of the eICU whiteboard with graphical representation of a patient's pipeline and warning symbols. Results: The eICU whiteboard in this study features imported clinical information systems, a unified interface design, and health-care-related information. The incidence of endotracheal tube extubation in the ICU decreased from 0.11% in 2014 to 0.0015% in 2016. Positive outcomes included time saved in 37.11% of cases, instantaneous responses to enquiries containing comprehensive information in approximately 29.56% of cases, and clear and easy to understand informational links in 21.38% of cases. This experience of constructing and implementing an electronic information system may serve as a reference for the development of medical information applications for use in health care institutions. Result: The proposed eICU whiteboard enables immediate access to patient-related medical care information. The whiteboard's ease of use and convenience can improve the effectiveness of clinical care.
