降低中醫針灸科門診病人遺漏針率

Reduce the outpatients' missing needle rate of Traditional Chinese Medicine Acupuncture Treatment Department

摘要

針灸治療為本院重要照護特色，而2014年共通報31件遺漏針，經實際觀察，調查2015年1月16日～2月15日期間發生遺漏針共24件佔0.33%，顯示問題的嚴重性。為有效降低遺漏針及提升病人安全，由專案小組進行檢討分析原因有人員出針後未做檢查、未核對計針板、新人經驗不足、醫師未用計針板、記錄不確實、計針板功能不足、衣物遮蔽等；遺漏針除造成病人生理傷害，醫護人員亦承擔心理壓力，故於2015年2月16日至8月15日為期6個月進行改善，設定專案目的為降低遺漏針率至0%，期間製作技術影片、針刺處置紀錄單，彩虹數針夾、舉辦出針護理宣導、修訂標準作業、評值表，經執行後稽核醫護人員出針正確率皆達100%、遺漏針率為0%，有效的提升病人安全與照護品質。

Acupuncture therapy is the one of our branch important specialties. In 2014, a total of 31 missing needles were reported. Therefore, from January 16 to February 15, 2015, the project leader directly went to the acupuncture clinic to conduct a direct observation method, and the reasons for the missing needles were registered. The number plus the actual number of leaked needles are counted in the statistics. The number of missing needles in a month is 24, the missing needle rate reaches 0.33%, show the severity of the problem. In order to reduce the missing needles efficiently and enhance the patients' safety, the project team analyzes major problems, including lack of double checking procedure after withdrawing the acupuncture needles, the unverified needle counting board, lack of experience of the new employees, doctor's incorrect record and not using the counting board, incomplete function of the counting board, and the private areas needles. Missing needle may harm the patients physically and increase the pressure of the medical staff; therefore, the project goal, from February 16th to August 15th, is to reduce the missing needle rate to 0% by making nursing education film, setting the precautions for withdrawing the needles and Standard Operating Procedure and Patient safety course training, revising the new employees' clinical practice training evaluation form, creating the needles position recording sheet and colorful needle counting clip. After the execution of the project, the needle withdrawal technique of the nurses and the accuracy rate of doctor's record of the missing needles have been audited. As a result, the accuracy rate is 100%, while the missing needle rate is 0%. This improves the patients' safety and the quality of care effectively.
