單一醫學中心之耳鼻喉頭頸部超音波檢查-10年經驗回顧

Head and Neck Ultrasound Examination-Ten-Year Experience from a Tertiary Referral Center

摘要


背景:超音波檢查廣泛地被運用於頸部腫塊的評估與治療。本研究的目的在回顧2007至2017年間,亞東醫院耳鼻喉頭頸外科進行頭頸部超音波檢查的經驗,提供給有興趣發展頭頸部超音波檢查的醫療人員參考。方法:回顧從2007年7月到2017年7月,約10年期間亞東醫院耳鼻喉頭頸外科進行之頭頸部超音波檢查,以及頸部腫塊於超音波導引下其細針穿刺或粗針切片的檢查結果。結果:10年來亞東醫院耳鼻喉頭頸外科超音波總共檢查13,922例,平均年齡為47歲,男性6,011名,女性7,911名。其中有6,828例為無穿刺檢查,約佔49%;使用超音波導引進行細針穿刺檢查共6,936例,約佔50%;以及使用超音波導引進行粗針切片檢查共158例,約佔1%。針對穿刺病因分析,接受細針穿刺的病因,以甲狀腺腫塊(45%)最多,其次為腫大淋巴結(22%),再其次為其他腫瘤(20%),最後為唾液腺腫瘤(13%);而接受粗針切片的病因則以腫大淋巴結(86%)為主。結論:頭頸部超音波檢查目前是評估頸部腫塊的第一線工具。耳鼻喉頭頸外科醫師可以將頭頸部超音波檢查更廣泛地應用於頭頸部病灶檢查。

BACKGROUND: Ultrasound examinations are widely used for the evaluation and treatment monitoring of neck masses. The purpose of this study is to review the experiences of head and neck ultrasound examinations in the Department of Otolaryngology and Head and Neck Surgery of the Far Eastern Memorial Hospital from 2007 to 2017. METHODS: We reviewed the head and neck ultrasound examinations from July 2007 to July 2017. We also reviewed the results from ultrasound-guided fine needle aspirations (US-FNAs) and core needle biopsies (CNBs). RESULTS: In the past 10 years, a total of 13,922 patients underwent head and neck ultrasound examinations in the Department of Otolaryngology and Head and Neck Surgery; the average age of the patients was 47 years old. There were 6,011 males and 7,911 females. Among these patients, 6,828 patients only received ultrasound examinations without aspiration or biopsy, accounting for 49% of the total patients; 6,936 patients received US-FNAs, accounting for 50% of the total patients; and 158 patients received US-CNBs, accounting for 1% of the total patients. The most common reasons for US-FNAs were thyroid nodules (45%), followed by lymphadenopathy (22%), other tumors (20%), and finally salivary gland tumors (13%). The most common reason for US-CNBs was lymphadenopathy (86%). CONCLUSION: Head and neck ultrasound examinations are the first-line tool for the evaluation of neck masses. Head and neck ultrasound examinations could be readily and extensively used for head and neck lesions by otolaryngology head and neck surgeons after proper training.
