應用倒頻譜分析評估嗓音異常疾患

Application of Cepstral Analysis for the Clinical Evaluation of Voice Disorders

摘要


背景:傳統的嗓音聲學評估是分析持續母音並量測音質參數的變異情形。近年來,利用倒頻譜分析方法所發展的平滑化倒頻譜突出峰值(Smoothed Cepstral Peak Prominence, CPPS),可應用於連續說話時的嗓音音質分析,是目前最受矚目的音質參數。本研究主要目的是應用倒頻譜聲學方法同時進行持續母音與連續說話的分析,了解CPPS在音質評估的量測表現,並進一步比較CPPS值與傳統聲學參數的量測差異。方法:嗓音異常病患經傳統母音聲學評估,只取三次Jitter值皆小於1之個案共112名,分別利用Praat及SpeechTool兩套語音分析程式套件,進一步將研究個案之三次持續母音與四個語句進行倒頻譜分析並記錄其CPPS值。結果:在控制Jitter值的情況下,持續母音的傳統聲學分析,除了振幅擾動參數值比一般建議稍微偏高,其他音質參數值都在建議正常範圍。在CPPS值部分,無論持續母音或連續語音,本研究之CPPSS_(peechTool)值與CPPS_(Praat)值大部分都位於現有研究建議之異常範圍。結論:本研究結果指出,有些病患的異常音質僅利用持續母音的傳統聲學分析方法,確實無法充分地呈現音質異常情形。透過倒頻譜分析之CPPS值在母音與語句的應用,能更多元呈現病患嗓音異常的情形。

BACKGROUND: A traditional voice evaluation analyzes continuous vowels and measures variation in voice quality parameters. Recently, the smoothed Cepstral Peak Prominence (CPPS) was developed using cepstrum analysis methods which can be applied to voice quality analysis of continuous speech. It is currently the most promising evaluation of voice quality. The aim of this study is to use cepstrum analysis for continuous vowels and continuous speech, to understand the outcome of CPPS in voice quality assessment, and to further compare measurement differences between CPPS values and traditional acoustic parameters. METHODS: Patients with voice disorders were assessed by traditional vowel acoustic analysis. Only 112 cases with three Jitter value measurements less than 1 were included in this study. Two speech analysis programs such as Praat and SpeechTools were used to further investigate three consecutive vowels and four sentences via cepstrum analysis. RESULTS: After controlling the Jitter value, traditional acoustic analysis of continuous vowels showed most voice quality parameter values were within the recommended normal range, except for shimmer and the amplitude perturbation quotient, which were slightly higher than the general values. The CPPSSpeechTool values and CPPSPraat values in this study were mostly in the abnormal range suggested by existing studies for both vowels and continuous speech. CONCLUSIONS: The results of this study indicate that traditional acoustic analysis of continuous vowels does not adequately represent abnormal voice quality in some patients. Through the application of cepstrum analysis to vowels and sentences, CPPS values can be used to show poor voice quality.
