目的:研究釹雅克(neodymium:YAG)雷射後囊切開術(posterior capsulotomy)對最佳眼鏡矯正視力(best spectacle-corrected visual acuity)的影響程度及其相關因素,以作為日後臨床應用時之參考。 方法:患有後發性白內障(after-cataract)的53隻眼(46人)接受為期三個月的追蹤,患眼在治療前以及治療後一週、一個月、三個月時,使用散瞳劑將瞳孔充份散大後,再依照自動驗光的結果來測量病患之最佳眼鏡矯正視力,並使用EAS-1000(上標 ®)(Nidek, Gamagori, Japan)眼前段分析儀測量雷射後囊切開之面積。 結果:在雷射後囊切開術後一週視力即有明顯的改善,這種情況可維持至少三個月;惟視力與後囊切開之面積大小、球面換算值(spherical equivalent)、總散光值(total refractive astigmatism)等等皆無統計上明顯的相關,但與剩餘散光(residual astigmatism)則有明顯相關。 結論:後發性白內障病患在接受雷射後囊切開術後視力會有明顯的進步,剩餘散光的減少可能有助於視力的改善。
Purpose: To evaluate the influence of neodymium:YAG laser posterior capsulotomy on the best spectacle-corrected visual acuity (BSCVA), and to find the relationship between BSCVA and various measurements. Methods: Fifty-three eyes with clinically significant after-cataract of 46 patients were included prospectively. Patients were examined under mydriasis before treatment as well as 1 week, 1 month, and 3 months after treatment. Measurements included refracttive status, BSCVA, and capsulotomy area. Results: The BSCVA gained more than 3 lines at 1 week after laser posterior capsulotomy, and that has persisted for at least 3 months. The BSCVA did not correlate with the capsulotomy area, spherical equivalent, and magnitude of total refractive astigmatism, but negatively correlated with the magnitude of residual astigmatism. Conclusions: Neodymium:YAG laser posterior capsulotomy significantly improved BSCVA soon within 1 week. The effect has maintained for at least 3 months and was associated with the decrement of residual astigmatism.
