Orbital Cellulitis Following Hordeolum in an Immunocompromised Patient with Diabetes and Breast Cancer-A Case Report

一位患糖尿病及乳癌免疫功能低下病患因麥粒腫導致眼窩蜂窩組織炎-病例報告

摘要


目的:報告一患有糖尿病及乳癌以致免疫功能低下的病患,因麥粒腫導致眼窩蜂窩組織炎。 方法:病例報告。 結果:一名45歲女性,因罹患麥粒腫後導致眼窩蜂窩組織炎。病患有糖尿病病史達13年,血糖一直控制不良;並剛完成乳癌的第8次化學治療,以致免疫功能低下。經立即給予靜脈注射抗生素治療以及麥粒腫膿瘍切開引流後,病況陝復良好。 結論:由於眼眶中隔具屏障作用,麥粒腫可以引起眼眶中隔前蜂窩組織炎,但很少侵入眼窩。但在免疫功能低下的病患,因劊曾加侵犯眼眶引起眼眶蜂窩組織炎的機會;因此,若有任何眼瞼感染時,快速且適當的治療是非常重要。

Purpose: To report a case of orbital cellulitis following hordeolum in an immunocompromised patient with diabetes and breast cancer. Method: Case report. Results: A 45-year-old woman, who has thirteen years of diabetes with poor glycemic control and just finished 8 cycles of chemotherapy for breast cancer, was presented with right orbital cellulitis following the occurrence of hordeolum. After immediate intravenous antibiotics treatment and drainage of the hordeolum abscess, the patient recovered well. Conclusion: As the orbital septum often serves as a barrier, hordeolum rarely induces a post-septal extension. But the risk of orbital cellulitis is increased in patients with suppressed immune system, and the importance of prompt and adequate treatment of any eyelid infection in such cases should be greatly emphasized.
