The Relationship between the Palmaris Longus Tendon and the Palmar Cutaneous Branch of the Median Nerve
中國人掌長肌腱與正中神經之掌皮支之解剖學變異

掌長肌腱在整形外科手術中是理想的肌腱移植考量，而種族上的變異常會影響手術的進行。在本文中，我們探討成年中國人的肌腱和正中神經之掌皮支相關性。我們統計(a)掌皮支橈側端到掌長肌的距離，(b)掌皮支尺側端到掌長肌的距離(c)正中神經之掌皮支起源到遠側腕關節皺摺的距離，(d)掌長肌腱部份到屈肌支持帶的距離。三年來一共統計了60具大體解剖，發現了120條掌皮支及109條掌長肌。結果發現，有88.3%的掌皮支起源於正中神經的橈側，其平均長度為1.4公分。有14個例子(11.7%)的掌皮支起源於正中神經的尺側，其平均長度為0.3公分。掌皮支起源到遠側腕關節皺摺平均距離爲3.2公分，而掌長肌近端肌腱部份到屈肌支持帶的平均距離為13.1公分。因此我們建議，和遠側腕關節皺摺距離四公分的橫向切口，是取掌長肌腱做肌腱移植的最好位置。

The palmaris longus tendon is an idea donor for tendon grafts for plastic surgery. Ethnological variations may affect surgical procedures. In this study, we evaluated the relationship of the palmaris longus (PL) tendon to the palmar cutaneous nerve of median nerve (PCN) in the Chinese adult. We measured the a) distance between the PCN radially to palmaris longus, b) distance between the ulnarward PCN to palmaris longus, c) distance between the origin of PCN to the distal wrist crease and, d) distance between the proximal tendon part of PL to distal wrist crease. A total of 60 cadavers with 120 PCNs and 109 PLs were examined in three consecutive years. 106 cases (88.3%) of the PCNs originated from radial side of the median nerve and the mean distance was 1.4 cm. 14cases (11.7%) of the PCNs originated ulnarward and the mean distance was 0.3 cm. The mean distance of origin of the PCN to the distal wrist crease was 3.2 cm and the mean distance of the proximal tendon part of PL to the distal wrist creast was 13.1 cm. Therefore, we suggest that transverse incision be made 4cm from the distal wrist crease in palmaris longus tendon graft.
