橈側多指症是上肢最常見的先天性異常,目前Wassel分類是最廣泛最簡單的分類方法。但是,Wassel分類太過於簡化,以至於無法適用於各種的變異,而且無法清楚的描述變異的型態是如何。所以,我們在Wassel分類的基礎上,增加兩個元素來改善Wassel分類上的缺點。 我們的分類法中第一個元素是多指分出的位置,如Wassel分類法一樣,但是特別定義Type Ⅶ是腕骨分出的多指。第二個元素是多指的特殊型態;譬如說triphalangism(記為Trip),rudimentary(記為Rud),多重多指(記為Mul)。第三個元素是描述多指的位置與內容物。 在我們的研究中,共收集了有63位橈側多指症的病患,71隻手。在42個病患身上實施了47個校正手術。
The radial polydactyly is commonly known as the duplicated thumb or the double thumb. Radial polydactyly is the most common congenital deformity of upper limb. The Wassel's classification is the most popular and the simplest method for thumb duplication. However, the Wassel's classification is too generous to apply to various deformities and it can't describe the deformity actually. Thus, we introduce a new modified classification of radial polydactyly, which is based on the Wassel's classification and the anatomic findings of the abnormal thumb. There are three components of our classification. The first one is the anatomic level of duplication as Wassel's method and we defined type Ⅶ as duplication at carpal bone level instead of triphalangism (Wassel's type Ⅶ). The second component is the specific duplication, such as triphalangism (record as Trip), rudimentary (record as Rud) and multiple duplicates (record as Mul). The third component is the location and content of duplicates. In our series of 63 patients with thumb polydactyly and 71 hands were evaluated totally. 47 surgical procedures for correction of thumb polydactyly were performed in 42 patients. We found that this modified classification is more appropriate to describe the anomaly in details than the original Wassel's classification, especially in the type Ⅳ and type Ⅵ conditions.
