Clinical Management for Advanced Heart Failure Patients with Ventricular Assisted Device (VAD)

摘要


由於器官捐贈短缺,各式機械型循環輔助器(mechanical circulatory support, MCS)已迅速發展,且廣泛應用於治療重度心衰竭。長期性心室輔助器(long-term ventricular assisted device, VAD)可以提供功能如,作為移植前之等待橋接期(bridge-to-transplant, BTT),等待恢復(bridge-to-recovery, BTR)或是永久療法(destination therapy, DT):皆有助於改善重度心衰竭患者之臨床狀況與生活品質。本篇文章著眼於介紹心室輔助器常見相關醫療狀況,以及患者立即性或長期性的併發症的臨床處置。

Because shortage of transplantable organs, widely range of mechanical circulatory support (MCS) devices have rapidly developed and applied to advanced heart failure patients. Long-term ventricular assisted device (VAD) can serve as bridge-to-transplant (BTT), bridge-to-recovery (BTR) or destination therapy (DT) to achieve the goal of improving advanced heart failure patients' clinical condition and quality of life. This article focuses on the common clinical management of long-term VAD and its immediate and chronic complications of the VAD patients.
