矯正牙齒的牙髓風險

摘要

矯正治療進行牙齒移動的同時，矯正的施力會造成牙髓及牙周的組織反應。在適當的矯正計畫與力量控制下，組織反應多侷限於暫時性的牙髓炎、有限度的根尖變鈍等等，但有少數的病患會產生不可預期的嚴重反應（如：牙髓根管閉塞、牙齒變色、牙髓神經壞死、嚴重根尖吸收以及牙根齒頸部外吸收…等），這不僅影響到牙齒的結構，美觀，進一步也可能會影響牙齒的癒後。因此，藉由回顧近年相關論文，針對在臨床上預測風險、處理矯正治療中的牙髓症狀以及後續的追蹤提出相關整理。

While moving teeth under orthodontic force, there are unavoidable tissue reaction due to the forces applied to the dental pulp and periodontal tissue. Under appropriate orthodontic treatment planning, most of the tissue reactions were limited to temporary pulpitis, blunting of dental root apex etc. However, a few patients will start up serious reactions such as dental pulp obliteration, tooth discoloration, dental pulp necrosis, or even severe apical root resorption and invasive cervical root resorption. These will not only have an impact on tooth structure, but will also affect the prognosis of the tooth. Therefore, by recalling the relevant papers in the recent years, we listed the relevant prediction of the clinical risks, the pulp symptoms during orthodontic treatment and the possible portal during follow up.
