提升首次居家化學治療病人攜帶式化療輸注幫浦照護之正確率

本專案目的為改善首次居家化學治療病人使用攜帶式輸注幫浦照護之正確率，降低病人返家後居家管路照護、活動注意事項及輸注問題等發生，並提升護理指導之完整性。居家攜帶式化療輸注幫浦大幅提升病人活動自主性，首次使用的病人返家後常電話諮詢照護問題，且經本院調查發現病人對居家化療輸注幫浦照護認知低，人員執行護理指導內容未一致性，因此引發專案改善動機。經問卷調查首次使用攜帶式化療輸注幫浦病人照護認知正確率僅63.2%、護理指導完整性為76.8%，故制定護理指導標準作業、說明簡易圖卡、增設實體教具，建立電話關懷機制。實施專案改善後，首次居家化學治療病人使用攜帶式輸注幫浦照護認知正確率提升為98.5%，護理指導內容完整性提升至96.4%，並使護理指導延續至居家增進病人自我照顧能力，對居家化療病人生活品質有顯著成效。

The purpose of this project is to enhance the accuracy of the use of portable chemotherapy for the first time home chemotherapy patients with a view to improving the integrity of nursing instruction after reducing the home care, activities and infusion problems. The first time use of patients after returning home often telephone counseling care problems, and the hospital survey found that the awareness of patients with home chemotherapy infusion pump care is low. In other words, the implementation of nursing staff to guide the content is inconsistent, thus triggering this project to improve. According to the questionnaire survey, the awareness of the first time use of portable chemotherapy infusion pump patient care is only 63.2% and nursing guidance integrity is 76.8%, so the development of nursing guidance standard operations is essential, including simple map cards, additional physical teaching aids, and the establishment of telephone care mechanism. After the implementation of the project, the awareness of the portable chemotherapy infusion pump care increased to 98.5%, and nursing guidance content integrity increased to 96.4%, thereby improving patients' self-care ability and the quality of life of patients with significant results.
