揭露癌症病人照顧者之過渡現象－過渡理論與實證文獻之交叉運用
Revealing Transitional Phenomenon in Caregivers of Cancer Patients-A Cross Application of Transition Theory and Evidence-Based Literature
摘要
家屬照顧者在癌症病患健康發生改變時，會引發個體經歷一個過渡的過程，正向看待過渡並協助他們成功地過渡，可促進個人健康的安適。本文以過渡理論為架構，透過系統性文獻查證來檢視照顧者的照顧經驗，綜合20篇研究結果發現：在照顧者間過渡是發生的，自家人有癌症診斷起，主要照顧者可能就開始發生情境性過渡，期間主要照顧者會面臨多元且複雜的事件，並出現五項過渡特性（知覺、參與、改變與差異、持續時間及關鍵點和事件），亦存在著個人、社區和社會的促進與抑制因子，在反應模式中以過程指標的因應較常被研究探討。藉由過渡理論和系統性文獻查證的交叉運用，可以讓實務工作者和研究者，以嶄新角度看待癌症病人照顧者的照顧議題，建議在實務上可加強對照顧者介入措施的提供，並評價介入措施的成效，建議發展質性研究以進一步了解照護者的過渡經驗，期能透過適當的介入方式，協助癌症病人照顧者正向地過渡。

Family caregivers experience a transition when cancer patients have changes in their health. Holding a positive view on transition and assisting patients and their caregivers through transitions successfully could enhance their well-being. Based on the framework of transition theory in combination with a systematic review of 20 studies, we found the transition occurred among caregivers. Once cancer is diagnosed, primary caregivers began to experience a situational transition. Primary caregivers had experienced multiple and complicated events during the caregiving period. Five properties (awareness, engagement, change and difference, transition time span, and critical points and events), facilitators and inhibitors in personal, community and society conditions were found across studies. In the patterns of response, coping was discussed more frequently than other process indicators. Application of transition theory and evidence-based literature allows practitioners and researchers to examine caregiver's issues with a refreshing view. In practice, we suggest to increase the interventions given to caregivers and to evaluate the effectiveness of interventions. We suggest qualitative research for enhancement of our understanding of the caregivers transitional experience. Using appropriate approaches to assist caregivers positively through transition process is expected.
