虛擬無對比劑影像在鑑別腦出血之應用-案例報告

Application of Brain Virtual Non-Contrast Images in Differentiation of Intracranial Hemorrhage- Case Report

摘要


本文之案例為心導管檢查後,初步診斷疑似急性腦中風(acute ischemic stroke, AIS)。因心導管檢查已施打對比劑,腦部組織與血管內對比劑已充盈,傳統單能量電腦斷層攝影難以準確區分是否為腦出血(intracranial hemorrhage, ICH),故執行雙能量電腦斷層攝影(dual energy computed tomography, DECT),由DECT掃描可獲得融合影像(mixed energy images, MEI)、虛擬無對比劑影像(virtual non-contrast image, VNC)與碘累加影像(iodine overlay map, IOM),用以鑑別ICH或急性阻塞性腦中風(acute thrombotic infarction)。

The case was diagnosed as suspected acute ischemic stroke after cardiac catheterization. Once the contrast was admitted through cardiac catheterization, brain tissue and vessels were filled up with contrast, making single energy CT hard to identify an intracranial hemorrhage; thus, dual energy computed tomography was applied instead. DECT scan can acquire mixed energy images, virtual non-contrast image and iodine overlay map to distinguish ICH or acute thrombotic infarction.
