MTA在治療無活性未成熟恆牙上的應用-文獻回顧及病例報告

The Use of MTA in Non-vital Immature Teeth: Review of Literature and Report of a Case

摘要


臨床上常見到根尖未關閉之恆牙,因外傷或齲齒等因素造成齒髓壞死,傳統的根尖成形術(apexification)是使用氫氧化鈣來誘導根尖鈣化屏障形成,以利於根管之永久充填。近年來因材料的進步而有了新的選擇:三氧礦聚合物(mineral trioxide aggregate, MTA)。由於其在密封性及生物相容性等方面優異的表現,在加上可縮短治療的時間,因而漸漸被注意並應用於根尖成形術。本病例報告為十一歲的男孩因中央牙阜斷裂造成齒髓暴露後壞死,經由根管消毒並以MTA在顯微鏡下進行開放根尖的填補,最後將牙齒復形後觀察追蹤,結果發現牙周組織漸漸恢復健康。本文並回顧相關文獻,包括最新的無活性未成熟恆牙血管再生(revascularizaion)的觀念,以提供臨床選擇處置方式的參考。

Pulp necrosis of the immature permanent teeth is common seen in pediatric dental clinic. In the past, apexification using calcium hydroxide is the most accepted procedure to induce the formation of a hard calcific barrier in the apex. Recently, mineral trioxide aggregate (MTA) has been shown to be a potential root-end filling material because of its superior sealing ability, biocompatibility, and short treatment duration. This article presents an 11-year-old boy with necrotic pulp of the mandibular left first premolar due to fracture of central cusp and subsequent pulp exposure. The root canal was disinfected and the apical portion was filled with MTA under microscope. This article also reviews pertinent literatures, including the new concept of revascularization and a different treatment protocol.
