FBSS (Failed Back Surgery Syndrome)泛指任何形式的腰椎手術後,腹部或下肢仍有持續的慢性疼痛,發生率高達百分之四十,在臨床上是非常常見的現象。1981年,Burton等人對此最早進行調查,他們認爲在全美每年進行的20萬例腰薦外科手術中,有20%-40%會發生FBSS,從此FBSS逐漸引起醫界的注意。 中醫臨床上亦經常接觸此類型病患,他們的臨床表現和一般的骨質增生、椎間盤突出,或是坐骨神經痛有些不同,因爲病患經過手術治療,正氣已大爲耗損,而且還會有瘢痕增生及腰椎不穩等等情況產生。我們整理文獻後,將FBSS的治療分類爲四大方法:中藥、針灸、手法及運動治療,在這篇文章中,將會討論這四個治療方式的適應證,施行細節及注意事項。
FBSS (Failed Back Surgery Syndrome) is a term that means persistent chronic low back pain and/or lower extremity pain after any type of lumbar surgery. Incidence of FBSS is up to 40%, and it is very common to clinical practice. In 1981, Burton et al investigated at the first time, they announced that 20-40% of two hundred thousand lumbar surgeries in America occurred FBSS. Therefore, FBSS was noticed by physicians. Doctors of Chinese medicine meet patients with FBSS frequently. The manifestations of patients with FBSS are different from osteoporosis, HIVD and sciatica because the right qi has been damaged after surgery. They also have problems of scar tissue formation and lumbar spine instability. We collect articles about the therapy methods of FBSS with Chinese medicine, including four methods: medication, acupuncture, manipulation and exercise. In this article, we will discuss the indications, detail of operations and cautions of these therapy methods.
