探討替代照法在直立式胸部X光攝影下心臟胸廓比之影響

摘要


本研究的目的在於評估在病患無法配合以標準姿勢受檢之情況下,採取替代照法取代傳統直立式胸部X光檢查的臨床可行性,並評估不同SID對病患心臟放大率的影響。在針對無法配合直立式胸部X光檢查的病患中,臨床上常採取由正面往背後照射(Anterior-Posterior)之替代照法,然而此一技術是否會造成病患影像會因為與心臟影像接收板之間的距離(OID, Object-to-Image Distance)增加產生心臟放大效應,進而影響到後續心臟與胸廓比(CTR, Cardiothoracic Ratio)量測上的結果誤差,導致病患在診斷心臟肥大(Cadiomegaly)診斷上有不同的結果,仍有進行進一步評估之必要。本實驗設計利用Chest PA Standing攝影並以標準SID180cm取得X光影像後取其為對照組,再以Chest AP Standing攝影並在心臟影像接收板之間的距離(OID, Object-to-Image Distance)相同之前提下,以不同SID取得X光影像後取其為實驗組,之後量測對照組各點距離並計算各張X光片之CTR值,再與實驗組數據以統計方式計算其P-Value皆大於0.05。表示不論SID距離在100~180cm且不論AP或PA方式照相,對於心臟肥大之診斷將不受影響。

The purpose of this study is to evaluate the effect of different SIDs on the magnification of the heart when the patient is unable to cooperate with the standard posture to be examined. Assuming that if an alternative method of illuminating from the front to the back (Anterior-Posterior) is adopted, the patient's image will be enlarged due to the increase in the distance (OID, Object-to-Image Distance) from the heart image receiving plate, which will affect In the follow-up measurement of the cardiac to thoracic ratio (CTR, Cardiothoracic Ratio), the results are incorrect, leading to different results in the diagnosis of cardiac hypertrophy (Cadiomegaly). This experimental design uses Chest PA Standing photography and obtains X-ray images with a standard SID180cm and takes it as the experimental group. Then takes Chest AP Standing photography and obtains X-ray images with different SIDs and takes it as the control group. Then, measure the distance of each point in the control group. And calculate the CTR value of each X-ray film, and then statistically calculate the P-Value with the experimental group data to be greater than 0.05. It means that the diagnosis of cardiac hypertrophy will not be affected regardless of whether the SID distance is 100-180cm and regardless of AP or PA mode.
