本文描述一位曾接受胃縫合手術卻再次復發的穿孔性消化性潰瘍個案之護理經驗。護理期間為2016年5月3日至5月12日,藉由觀察、會談、傾聽、身體評估及病歷查閱,收集主、客觀資料,運用Gordon 11項健康功能型態評估,確立健康問題有急性疼痛、潛在危險性感染、焦慮與遵從性差等;照護過程中,給予個別的護理指導,評估傷口疼痛情況並減輕不適;藉由醫療團隊的病情治療與預後解說,適時澄清個案心中疑慮,協助面對疾病復發,增加對疾病的認識,進而減輕焦慮;介紹術後成功病友,藉探視與生活經驗分享,增進個案自信心,減少對疾病預後之不確定感;深入了解個案不正確飲食習慣,採取健康生活模式,提升自我照顧的能力。照護期間個案的焦慮視覺類比量表分數由85分降至60分、疾病不確定感量表分數由92分降至59分,顯示護理措施介入後能降低個案之焦慮與疾病不確定感;盼藉此護理經驗,提供護理同仁照護類似個案之參考並提升護理品質。
This article described the nursing care for a man who underwent surgical treatment for recurrent perforated peptic ulcer. The period of care was from May 3 to 12, 2016. The patient was assessed using Gordon's 11 Functional Health Patterns, with data collected and analyzed from observation, interview, physical evaluation, and medical records review. Health problems identified included acute wound pain, high risk of infection, anxiety, and compliance to health care. We established a trusting nurse-patient relationship by listening, supporting, and consulting to help the patient face his disease and build a support system. During the nursing period, the patient's anxiety visual scale decreased from 85 to 60 and his uncertainty scale of disease index decreased from 92 to 59. After nursing interventions, the patient was able to positively adjust to reduce uncertainty and gain knowledge to enhance self-care ability and adaptability, improving his quality of life. This nursing experience can be a reference for similar patients to improve the quality of nursing care.
