單中心服用華法林患者抗凝治療範圍內時間及現狀分析
Analysis of time in therapeutic range of anticoagulant therapy in patients on warfarin a single-centred study
摘要
目的：分析本院接受口服華法林抗凝治療患者的治療範圍內時間（Time in Therapeutic Range, TTR）及抗凝現況，為華法林抗凝臨床應用管理提供參考。方法：通過收集由2016年6月～2020年12月（共4.5年）在本院接受華法林抗凝治療的患者資料，納入符合研究標準及排除標準的患者後，根據患者臨床資料及國際化標準比值（International Normalized Ratio, INR）計算TTR，並對出血事件、栓塞事件、死亡事件等相關不良事件行回顧性分析。結果：納入接受華法林抗凝治療患者共384人，經規範隨訪定期檢測INR的患者為122人，其中INR檢測密度達標者為90人，其平均TTR為54.09±21.56%；抗凝治療品質達標者佔40%。結論：研究結果提示本院整體服用華法林患者INR定期隨訪質量並不理想，但在INR檢測密度理想情況下，其TTR平均水平接近達到滿意水平，臨床上可進一步加強對華法林抗凝的整體管理和規範化。

Objective: To analyze the Time in Therapeutic Range (TTR) and anticoagulation status of patients receiving oral anticoagulation therapy with warfarin at Kiang Wu Hospital and provide further information for the clinical application and management of warfarin . Methods: Collect the data of patients who received anticoagulation therapy (warfarin) in our hospital from June 2016 to December 2020 (4.5 years in total), and after enrolling patients who met the research criteria and exclusion criteria, according to the clinical data and International Normalized Ratio (INR) of the patients, these data were used to calculate TTR, and retrospective analysis of related adverse events such as bleeding events, embolic events, and death events was performed. Results: A total of 384 patients receiving warfarin anticoagulation therapy were included. 122 patients received regular INR testing after follow-up. Among them, 90 patients met the INR test density, and their average TTR was 54.09±21.56%; the quality of anticoagulation therapy met the standard. They have accounted for 40%. Conclusion: The study results suggest that the quality of regular follow-up of INR for patients taking warfarin in our hospital as a whole is not satisfactory. However, when the patients' INR detection density is ideal, their average TTR is approaching satisfactory. The overall anticoagulation therapy with warfarin needs to be strengthened, including management and standardization in practice.
