Respiratory Management and Ethical Issues of an Infant with Type I Muscle Fiber Atrophy

Muscle atrophy is the wasting or loss of muscle tissue. We report a case of female prematurity with generalized muscle weakness followingbirth and intubation in the delivery room. Severe muscle weakness and markedly delayed developmental milestones from early infancy were noted. Biochemical and morphologic studies of biopsied muscles disclosed type 1 fiber atrophy, which was quite different from patients with spinal muscular atrophy and congenital muscular dystrophy. In this patient, the disease was not progressive. Although no confirmed diagnosis was made in this patient, we also discuss the indications for tracheostomy in pediatric patients and the ethics of decision making in end-stage lung disease.
