精密傳統印模法對動搖齒位移及精確度的影響

The Displacement and Accuracy of Conventional Impression Methods for Mobile Teeth

摘要


目的:由於牙周病患者進行全口贋復重建的需求與日俱增,真實地轉移口內動搖齒位置至技工室將是一個重要的課題。本研究設計一個模擬牙周病動搖齒的模型,研究不同稠度的精密彈性印模材對於動搖齒造成位移的影響,希望可以提供牙周病科醫師對動搖齒進行贋復前的治療目標。材料與方法:在一級及二級牙齒動搖度(tooth mobility Class I & Class II, Miller's mobility index)的實驗模型,使用三種不同稠度的加成式矽膠印模材(additional silicone),分別以(1)單相印模法(mono-phase one-step single-mix impression technique),(2)高流動性與中流動性之雙相一步印模法(dual-phase, light body and monophase, one-step double-mix impression technique)與(3)高流動性與低流動性之雙相一步印模法(dual-phase, light body and heavy body, one-step double-mix impression technique)三種方式,使用個人印模托(individual tray)進行傳統印模。結果:三種傳統印模方式皆造成一級及二級動搖齒有頰側位移的現象,唯以高流動性合併中流動性之雙相一步印模法之精密彈性印模材組合對於牙周病有動搖度牙齒所造成的頰側位移量最小。結論:傳統精密印模對牙周病動搖牙齒都會產生部分的位移,故如果以傳統印模法翻印具動搖度支台齒之跨弓治療時,牙周病二級動搖齒建議持續使用治療用臨時性假牙,追蹤觀察直到動搖齒牙周治療恢復到一級動搖度後,再以高流動性(light body)合併中流動性(monophase)之印模材組合進行最終印模,才能減少裝戴牙齒的誤差。

Purpose: As the proportion of mobile tooth impression has gradually increased, precisely transferring the mobile tooth to the laboratory will be an important issue. In the study, we designed an experimental model that simulates chronic periodontitis mobile tooth and using different viscosity of elastic impression material to observe the possible displacement during conventional impression technique. It is hoped that the results of this study will provide periodontists a reference for pre-prosthetic treatment goals of mobile teeth. Materials and methods: The model simulates periodontal mobile tooth with tooth mobility Class I & Class II, Miller's mobility index and take a final impression with one-step single-mix impression technique and one-step double-mix impression technique by using the custom tray, and observed the displacement with three different consistencies, respectively, along with 3 kinds of impression material permutation of monophase (Honigum-mono, DMG, Germany), light body combine monophase (Take-1 Advanced light body and Take-1 Medium/Monophase, Kerrdental, USA) and light body combine heavy body (Aquasil Ultra XLV, and Aquasil Ultra Heavy, Dentsply Sirona, USA) additional silicone impression materials. Results: All impression methods have buccal displacement on the conventional impression for both tooth mobility Class I and mobility Class II teeth. However, the method of one-step double mixing impression technique combine light body with monophase additional silicone shows the least the buccal displacement for both mobility I and mobility II teeth in the conventional impression technique. Conclusion: All conventional impression techniques with additional silicone impressions material will cause mild to moderate displacement for periodontal diseased teeth. Therefore, if the conventional impression method is used to reproduce the cross-arch model of the mobile abutment teeth of tooth mobility Class II must be continuously treated with provisional prostheses until the mobile teeth return to mobility Class I, then take final impression combines light body with monophase to reduce the error of prostheses try-in.
