多發性膽管缺陷瘤－病例報告

多發性膽管缺陷瘤為一罕見良性病灶，在肝臟內有多發性小的結節，1918年由von Meyenburg首先提出，發現肝囊腫病例可併發成團的肝內膽管病灶，其稱之為von Meyenburg's complex（VMC），以後描述此種病灶所用的同義詞甚多，目前大多以多發性膽管缺陷瘤或VMC稱之。此病一般都是偶然發現，臨床上易誤診為惡性腫瘤，所以組織病理診斷非常重要，目前都認為此種病灶並非真正的腫瘤，而是一種缺陷瘤，表現出不正常發育的膽管。本報告病例具有典型的病理變化，故提出討論並做文獻回顧。

Multiple bile duct hamartomas of the liver are comparatively rare. In 1918, von Meyenburg reported 12 cases of cystic liver with isolated groups of intrahepatic bile ducts which he called ”complex”. Since then, many synonyms were used to describe this entity. At present, multiple bile duct hamartomas and von Meyenburg's complex (VMC) are most commonly used. They are usually samll and multiple nodules and are usually encountered as incidental findings at surgery or autopsy. Their primary clinical significance is the fact they are easily confused with metastatic foci of carcinoma on gross examination; however, the microscopic findings are characteristic. It is generally accepted that they are hamartomatous and represent developmental anomalies of bile duets and are not true neoplasms. This reported case is also encountered incidentally and show typical findings of multiple bile duet hamartomas on gross and microscopic examinations. The literatures are reviewed.
