青少年常見之骨骼肌肉系統症狀

摘要


由於國內青少年升學壓力極大,針對青少年因壓力造成之骨骼肌肉系統不適症狀所作的廣泛性調查十分重要。本篇文章的主要目的乃在針對不同的身體區域,探討何處是青少年最易產生骨骼肌肉系統症狀之所在;作者亦同時分析不同區域骨骼肌肉系統不適之肇因是否不同。本次研究以問卷的方式,針對台南地區的高中生做調查。受測者包括三十一位男性學生及四十二位女性學生。結果顯示,在骨骼肌肉系統不適的發生位置方面,最容易發生的身體部位分別為:頸部(79%)、肩部(70%)、及背部(44%)。女性學生的結果和整個族群類似;但是男性學生最容易發生的身體部位則為:頸部(74%)、肩部(61%)、及腳踝(45%)與手腕(45%)。針對發生部位的症狀作分析,各部位都是以酸痛為主。分析造成骨骼肌肉系統不適原因,在身體上半身部份:頸部、肩部、及背部以姿勢不良為主要造成之原因。在病後尋求治療方面,大部份的同學以休息為主要處理方式。這個調查可以估計骨骼肌肉系統症狀在青少年族群的發生率。而物理治療師也可擔任臨床檢查前的篩選工作。如此,不僅可以藉由早期介入降低醫療成本,也可預防青少年族群將來因骨骼肌肉系統症狀而造成累積性傷害或失能等現象。

Musculoskeletal symptoms usually occur to the adolescent population. These symptoms can not be neglected because they might contribute to cumulative trauma diseases(CTDs) in the future. The purpose of this study was trying to explore which is the most common body part that musculoskeletal symptoms occur to this population and also to analyze the contributing factors that cause these symptoms in adolescence. A questionnaire in Chinese was developed by the author. The sample included students of several senior high schools in Tainan area. A total of 80 questionnaires were distributed to these students. Data were analyzed using SPSS software. Descriptive statistic were computed for means, standard deviations and frequencies. Seventy-five of the questionnaires were returned finally. There were two questionnaires with incomplete data sets, therefore, 73 of them were analyzed, including 31 male and 42 female students. For the frequency analysis, according to the body part diagram, the leading three areas of musculoskeletal symptoms in this population were: neck area(79%), shoulder area(70%), and back area(44%). The leading areas for girls were the same as the whole population while those for boys were different. The leading four areas of musculoskeletal symptoms for boys were: neck area (74%), shoulder area(61%), ankle(45%), and wrist (45%)area. Of students with symptoms, there were 75% with soreness, 5% with numbness, and 3% with tingling in the neck area. Among the contributing factors related to these musculoskeletal symptoms, awkward posture is the most important one(71%) in neck area, while application frequency(13%)follows. The results showed that the incidence of musculoskeletal symptoms is quite high in adolescent population. However, lack of CTD prevention concept is common in this population, and most of them do not know how to appropriately manage these symptoms. The investigator concluded that the surveillance of this population can estimate prevalence and, potentially, preclinical detection of these disorders to permit early intervention, medical costs, and to minimize disability in the future. Education of CTD concept may be beneficial for this population.
