Hypercalcemia due to hyperparathyroidism or other metabolic disorders is recognized as a cause of neurological disturbances, including dementia and parkinsonism. Thus, a hypercalcemia-induced cytotoxic effect in the central nervous system may play an important role in neuropsychiatric manifestations. However, there have only been rare prion reports of primary hyperparathyroidism manifestating as dementia. We report a patient who was noted to have progressive dementia for three years. Hypercalcemia was later noted in routine biochemical evaluation. After removal of a parathyroid adenoma, mentality returned to normal status and dementia disappeared completely.
