Quantitative Cardiac Computed Tomography Evaluation of a Premature Baby with Transposition of Great Arteries: A Case Report

摘要


一位137天之早產嬰兒,臨床上被診斷為大動脈轉位,心房中隔缺損和開放性動脈導管在接受了左心室準備性手術(包括左側Blalock-Taussig分流術和肺動脈箝縛術)之後,吾等利用超快速電腦斷層掃描攝影之檢查,以作為定量評估心臟功能的另一途徑。患者在當時因為有下腔靜脈阻塞,限制了心導管的檢查。另外,雖然心臟超音波能有效的評估左心室壁的厚度,但由於患者心臟呈現異常性的幾何形狀,因而對於左心室大小的估算僅只能視為補充參考之用。然而超快速電腦斷層掃描攝影,對於心室大小以及質量上的量度,有著既明瞭又正確的闡述,可以克服有不規則幾何形狀的心臟測量上之困難。

Quantitative ultrafast computed tomography (UFCT) was used for alternative study of cardiac function in a 137-day-old infant, a premature baby with clinical diagnosis of transposition of great arteries (TGA), atrial septal defect and patent ductus arteriosus, after left-ventricle-preparation surgery including left Blalock-Taussig shunt and pulmonary arterial banding. The patient had inferior vena cava thrombosis, which limited use of cardiac catheterization at that time. Echocardiographic evaluation validated the thickness of left ventricular wall, and was only complementary to assessment of the left ventricular size because of the unusual geometry of the patients' heart. The measurements for the ventricular sizes and masses from UFCT delineated clearly and precisely in spite of their irregular geometry.
