懷孕後期致急速失明之鞍部腦膜瘤―病例報告

摘要

本案例敍述一位第二交懷孕的31歲婦女，在懷孕第三期時出現右眼顳側偏盲，視力急遽喪失。追溯其病史，病人在前一次懷孕，亦在懷孕第三期出現左眼視力急遽喪失，以致左眼失明，視神經萎縮。腦部核磁共振檢查發現一巨大鞍部腫瘤。病人在高劑量類固醇以提高胎兒肺成熟度，並減少腦水腫的治療後，自然順產，並作腫瘤切除。產後視力逐漸恢復，在腫瘤切除後，右眼視力由眼前辨指數，恢復到0.8。病理發現為腦膜瘤。腦膜瘤在懷孕時期會急速增大，而出現神經症狀，顯示腦膜瘤的發生與增大，受女性荷爾蒙的影響，在本案例得到印證。

A young woman in the third trimester of her second pregnancy presented with a rapid onset of temporal hemianopia on the right side and reduced visual acuity. Tracing back her history, in the third trimester of her first pregnancy, the same episode occurred in her left eye resulting in blindness and optic atrophy. The X-ray finding on MRI revealed a big suprasellar tumor. A short course of high-dose steroid was given to promote fetal lung maturity and relieve brain edema. After giving birth, the visual acuity got improved, then operation with tumor resection was performed. The visual acuity of right eye became 0.8 within two months compared to that of counting finger before delivery. The pathological findings showed a meningioma. The meningioma showed rapid growing during pregnarcy indicating correlation with the sex steroid hormone.
