Association between Symptoms/Diagnosis of Attention-deficit/Hyperactivity Disorders and Sleep Problems/Disorders-focus on Taiwan's Studies

摘要

目的：注意力不足過動症和睡眠問題／疾患的症狀及其所造成的行為問題常常是很相似的，而這二個問題也容易共病而造成更嚴重的功能損害，本綜說將回顧注意力不足過動症和睡眠問題／疾患二者之關聯性。方法：有系統地回顧發表於SCI/SSCI的相關文獻。結果：文獻回顧結果顯示不專心及／或過動的症狀及注意力不足過動症的診斷和一些睡眠問題／疾患是有相關的，尤其是睡眠中周期性肢動症、不寧腿症候群、睡眠呼吸障礙及失眠，但和類睡症無關。藥物使用及其他共病的精神疾患都會影響上述的相關性。結論：在評估臨床表現有不專心及／或過動症狀的病人時，需要注意其是否有睡眠問題／疾患及其他共病的精神疾患與藥物的副作用，以減少在這群病人不正確的診斷及不當的處置其睡眠問題。

Objective: Symptoms and consequences of attention-deficit/hyperactivity disorder (ADHD) and sleep problems frequently overlap, and both of them complicate the impairments which merit attention. This overview is to summarize the association between the sleep problems/disorders and ADHD symptoms/diagnosis, particularly focusing on recent studies in Taiwan. Method: Literature concerning this topic published in SCI/SSCI journals was systemically reviewed. Result: ADHD-related symptoms and the diagnosis of ADHD are associated with some sleep problems/disorders, such as periodic limb movements during sleep, restless legs syndrome, sleep-disordered breathing and dyssomnia, but not with parasomnia. Use of medication and other psychiatric comorbidities are potential confounders. Conclusion: Assessing sleep problems/disorders among patients with ADHD symptoms and screening for other psychiatric comorbidities and the side effects of medication are important to avoid inappropriate diagnosis and inadequate management of sleep problems/disorders among patients with ADHD.
