台灣跨專業共識之化學治療安全作業指引

本安全作業指引提供有關處方、備藥／發藥、和給藥安全上的建議，目的為了預防化學治療相關的給藥錯誤和提升病人安全。這個作業指引是根據目前最佳的證據和專家的意見並藉由多專業協同合作所建立的共識，其中包含七大項目：1）資格與訓練；2）進行化學治療前需完成的病人評估或病歷記載；3）化學治療給藥前的病人評估；4）處方要素；5）藥物製備；6）化學治療給藥；7）居家化學治療。本指引提供建議給多專科團隊成員包括醫師、護理師、藥師等參與抗癌瘤藥物給藥者，然而本作業指引並不取代臨床專業的判斷。

The purpose of these safety guidelines is to provide recommendations on the process of prescribing, dispensing and administering chemotherapy for cancer patients to prevent medication errors and improve patient safety. These guidelines, based on the best available evidence and expert opinion of professionals, were developed by using multidisciplinary, consensus building process. The resulting guidelines are categorized into seven domains: 1) staff qualification and training; 2) preparation for chemotherapy- patient assessment and documentation; 3) patient assessment prior to each cycle of chemotherapy; 4) required elements of chemotherapy orders; 5) drug preparation; 6) administration of chemotherapy; 7) home chemotherapy. These guidelines are for multidisciplinary readers including medical, nursing and pharmacy staff involved in delivering anti-cancer drugs.
