The purpose of these safety guidelines is to provide recommendations on the process of prescribing, dispensing and administering chemotherapy for cancer patients to prevent medication errors and improve patient safety. These guidelines, based on the best available evidence and expert opinion of professionals, were developed by using multidisciplinary, consensus building process. The resulting guidelines are categorized into seven domains: 1) staff qualification and training; 2) preparation for chemotherapy- patient assessment and documentation; 3) patient assessment prior to each cycle of chemotherapy; 4) required elements of chemotherapy orders; 5) drug preparation; 6) administration of chemotherapy; 7) home chemotherapy. These guidelines are for multidisciplinary readers including medical, nursing and pharmacy staff involved in delivering anti-cancer drugs.
