摘要

唇顎裂的患者視病灶部位和範圍大小，除了對顱顏面美觀，在吞嚥及語言功能上也會有不同程度的影響。對於無法接受手術治療的患者，上顎封閉器贗復物可設計用於幫助解決病人於語言，咀嚼及美觀上的問題。本病歷報告針對一位46歲患單側顎裂，且具有嚴重牙周病及齲齒問題之男患者，紀錄如何製作一個全口活動假牙結合上顎封閉器，重建病人美觀、說話及咀嚼的功能，並恢復患者長期以來所缺乏的自信心及社交能力做一討論，旨能針對類似的唇顎裂患者之牙周贗復的療程提供參考。

Objective: Most patients with cleft lip and palate have problems with eating and speech. Obturator is a prosthesis designed to close up the defected palate to aid speech, mastication, esthetics, and proper air flow. Previous studies have suggested that selection of proper material and tray are crucial to obtain optimal treatment outcome for patients with cleft lip and palate. We reported a 46 year-old male patient with cleft palate accompanied with multiple dental complications and the procedures to restore his speech and mastication function with a complete denture in combination of an obturator. With a sequential rehabilitation protocol including the surgical reduction of posterior flanges to lessen gag reflex reaction, his symptoms were relieved and the treatment outcome was satisfactory.
