母親透過錄影帶蘊育其與早產新生兒之依附關係的經驗歷程

Using Video to Explore the Bonding Process between Mothers and Their Premature Newborns

摘要


本研究目的是探討藉由早產兒餵食之錄影帶協助母親與其早產新生兒建立依附關係之經驗歷程。研究設計是採現象學研究法,研究者拍攝早產兒於新生兒加護病房餵奶時的錄影帶,再與早產兒母親觀看錄影帶時進行一對一之深度訪談。研究者將十位早產兒母親看錄影帶時所表達的語言行為,寫成敘述體的文稿,再以Colaizzi分析法分析,進一步歸納整理,並對其意涵加以歸類及命名。研究結果發現,透過錄影帶母親與其早產新生兒建立依附關係之經驗歷程,可歸納為「鉅細靡遺的關注行為」、「比對家人的確認行為」、「修正印象的比較行為」、「比對標準的評價行為」、「滿足需求的互動行為」、「提前準備的母情行為」。本研究的結果,可協助護理人員瞭解早產兒母子「聚少離多」期間依附關係的建立過程,且可提供以家庭為中心的護理參考。

The purpose of this study was to explore the bonding process between mothers and their premature newborns through videotape. A phenomenology method was used to design the study. The researcher made video recordings of each newborn at feeding time in the neonatal intensive care unit (NICU) before conducting an in-depth interview with the mother as she watched the video. Transcription of the mothers' verbal behavior was performed and the transcript analyzed using Colaizzi's phenomenological methodology. A purposive sample of ten premature mothers participated in this study. Six themes emerged from the interview data, including: behavior of close observation and concern, affirmative behavior comparing with the characteristics of family members, comparing behavior (revising impressions), evaluative behavior (making comparisons with standards), interactive behavior of supplying needs, and maternal behavior of beginning preparation. The findings of this study may help nurse professionals understand the attachment of mothers and their premature newborns in the period of ”short contact and long separation” and may be applied as references in family-centered care.
