台灣老年人接種季節性流行性感冒疫苗的相關因子-2009年國民健康訪問暨藥物濫用調查

Factors Associated with Seasonal Influenza Vaccination among Elderly in Taiwan-Analysis of the 2009 National Health Interview Survey

摘要


目的:本研究旨在探討台灣老年人接種季節性流行性感冒疫苗之相關因子。方法:本研究的資料來源為衛生福利部國民健康署、財團法人國家衛生研究院及衛生福利部食品藥物管理署提供之2009年「國民健康訪問暨藥物濫用調查」原始資料檔。以65歲以上受訪者為研究對象,剔除訪員觀察受訪者心智狀況為異常或不佳者,以過去一年間是否曾接種流行性感冒疫苗作為依變項,進行羅吉斯迴歸分析其相關因子。結果:經加權處理的有效樣本2,399名65歲以上受訪者中,過去一年間曾有接種流感疫苗者共1,203人(50.1%)。羅吉斯複迴歸分析發現社區老人接種流行性感冒疫苗與8項因素呈現顯著獨立關聯。該8項因素包括年齡為75歲或以上(勝算比[OR]=1.92, p<0.001)、與配偶同住(OR=1.47, p<0.001)、現居地為鄉或鎮(OR=1.23, p=0.020)、過去一個月有使用西醫門診(OR=1.40, p<0.001)、過去一年曾有接受健康檢查(OR=2.62, p<0.001)、過去兩週有運動(OR=1.21, p=0.035)、目前沒有吸菸(OR=1.70, p<0.001)和有參加團體活動(OR=1.28, p=0.022)。結論:台灣老年人接種季節性流行性感冒疫苗與多項因素呈顯著獨立關聯。本研究之結果可作為日後計畫社區老年人流感預防策略與介入之參考。

Objectives: To investigate the factors associated with seasonal influenza vaccination among the elderly in Taiwan. Methods: We conducted a secondary analysis of the data obtained from the 2009 National Health Interview Survey in Taiwan, provided by the Health Promotion Administration (HPA) and Food and Drug Administration (FDA), Ministry of Health and Welfare (HPA) and National Health Research Institutes (NHRI). Individuals aged 65 years and above were included in the analyses. Individuals with noticeable cognitive impairment, as determined by the interviewers, were excluded. Logistic regression analyses were conducted using the variable 〞whether the respondent had received influenza vaccination in the past year〞 as the dependent variable. Results: Of the 2,399 respondents (sampling weight-adjusted), 1,203 (50.1%) had received influenza vaccination in the past year. Results from the multivariate logistic regression analysis identified the following eight factors significantly associated with the uptake of influenza vaccination: aged 75 years or above (odds ratio [OR]=1.92, p<0.001), living with a spouse (OR=1.47, p<0.001), having outpatient visits in the past month (OR=1.40, p<0.001), having a health examination in the past year (OR=2.62, p<0.001), having exercises in the past two weeks (OR=1.21, p=0.035), not a current smoker (OR=1.70, p<0.001), living in a rural township (OR=1.23, p=0.020), and participating in group activities (OR=1.28, p=0.022). Conclusion: A number of factors were significantly and independently associated with seasonal influenza vaccination in elderly Taiwanese. Our findings could serve as a basis in planning prevention and intervention strategies for elderly community dwellers in Taiwan.
