Enhancing the Accuracy of Respiratory System Assessments among Staff Nurses in a Chest Medical Unit

摘要

護理評估是護理人員臨床工作中十分重要的工作，執行良好的護理評估，才能早期發現病患問題，維護病患安全、提升照顧品質。本專案旨在提升胸腔內科病房護理人員執行呼吸系統評估之正確率。經現況資料收集與分析，發現護理人員執行呼吸系統評估正確率偏低，僅49.1%，其呼吸系統評估的認知及技能不足，除可能會疏忽早期的呼吸異常變化外，亦無法早期發現病人呼吸系統的問題，影響病患照顧品質，故提出改善方案，包括：舉辦呼吸系統評估床邊教學、課堂及影音光碟教學、製作呼吸系統圖示口袋版。於改善方案執行後，護理人員執行呼吸系統評估正確率提升至93.5%；對呼吸系統評估的認知正確率提升至92.3%。本專案措施確實可改善護理人員執行呼吸系統評估之正確率及提升臨床照護品質，故建議可將此改善方案繼續推展於臨床實務中。

Nursing assessment is very important for staff nurses in clinical nursing. If staff nurses are able to perform nursing assessment accurately, they can early detection of disease problems in order to maintain patient safety, improve the quality of nursing care. This project was designed to improve the accuracy of respiratory system assessments among staff nurses in a chest medical unit. The data revealed that accuracy rate of respiratory system assessments was low, only about 49.1%. In these project strategies for improving the accuracy rate included beside teaching, lectures and video teaching, the development of respiratory assessment pocket chart. The accuracy rate was then increased to 93.5% and cognizance scores was increased to 92.3%. Overall this project was effective and may be used continuously for improving nurses' skills in the clinical practice. Enhance the quality of clinical care.
