陽和湯加減治療下肢傷口久不癒合之中西醫會診：一病例報告

摘要

外科名方「陽和湯」出自清代王維德《外科證治全生集》中，此方具溫陽補血、散寒通滯功用，歷代醫家譽為外科陰疽之祖方，對於諸陽虛寒凝之證，能散陰凝致陽和，故以陽和名之。本病例患者因蜂窩性組織炎入院治療，然傷口癒合不佳，且住院期間之實驗室檢查，未見白血球增加，細菌培養亦無特殊發現，加之病患對於「盤尼西林」過敏，故會診中醫治療。經辨證此患者屬陽虛寒凝證，病候屬陰疽範疇，給予陽和湯加減，傷口漸癒合，全身不適症狀減輕，終能出院休養。透過此中西醫會診病例報告，期能對於傷口久不癒合之病患，給予中醫治療上的參考。若能掌握「陽虛寒凝」之病機，相信陽和湯亦可廣泛應用於臨床上許多難解之各類疾患。

This patient suffered from left-leg swelling and pain at first. She was diagnosed with cellulitis by orthopaedic doctor and being hospitalized for further treatment. However, her surgical wound is bad healing during the first hospitalization period that she has already experienced many times debridement including drainage and skin-grafting. Therefore, the orthopaedic doctor consulted Chinese medicine doctor to combine care. We prescribed modified Yang-Ho-Tang to this patient. After one month treatment, she has the good wound healing and finally she can discharge from hospital. In this report, we will describe this patient’s treatment process and discuss about the indication and efficacy of the Yang-Ho-Tang.
