摘要

本研究目的為探討「預防尖銳物傷害之工作模式」的建立對手術室護理人員使用尖銳物之知識、技術與態度的影響。採同一樣本前後測的準實驗設計，以六個月之「預防尖銳物傷害之工作模式」為實驗處置。於一所2000床之醫學中心進行資料收集，研究對象為該手術室配置之護理人員，共58名。研究工具為依據文獻之自擬結構問卷，內容包括手術室護理人員使用尖銳物之知識、技術與態度量表，經專家效度檢測，及以尖銳物傷害之發生率為效標關聯效度，測得每週發生率由改善前0.83％降到改善後0.25％，並測內在一致性Cronbach's α為.82，間隔二週之再測信度測穩定性為.89。研究結果發現尖銳物傷害之工作模式的建立，對手術室護理人員之知識與技術有所改進，但態度方面之改變未具顯著性。知識與技術方面，增加得分的項目均包括多項平時工作中易被輕忽的事項。態度方面，增加得分的項目為自認為有信心可以做到避免尖銳物的傷害。研究對象之態度與技術、態度與知識之相關性分別為.274及.412，具顯著性。知識與技術之相關性為.188，不具顯著性。而研究對象之年齡與其技術呈正相關，但與其知識呈負相關。建議手術室行政管理上可推廣建立此模式，且於模式建立後仍要繼續維持宣導及查核，並不斷改善工作環境。

The purpose of this study was to evaluatethe effect of a sharp injury preventive model forthe operating room nurses.The setting was a 2000-bed medical center in which there are 2000 operationsperformed per month.Under a quasi-experimental design,a sharp injury preventive modelwas implemented for six months.There werepre-test and post-testevaluation results.The instrument of evaluation was a structured questionnaire,including knowledge,skill and attitude toward injury prevention which were tested usingcontent validity and compared with the frequencyof sharp injury for criterion-related validity.Cronbach's α was.82,and test-retest reliability was.89.The results showed that the scores on theknowledge and skill evaluation tests significantyimproved after implementation of the sharp injurypreventive model.However,the evaluation scoreson attitude did not show significant improvement.Most of the improvement in knowledge and skillconcerned the negligence in everyday routines.The improvement in attitude showed in the greaterconfidenceof the nurses in sharp injury prevention.The correlation of.274 between attitude and skill,and.412 between attitude and knowledge,respectively were significant.However,the correlationbetween attitude and skill was not significant.Wefound a positive correlation between skill andage,but negative correlation between knowledgeand age.According to the data,we suggest thathospital administration should set up preventivemodel,and continuously monitor the knowledge,skills and attitudes of the nurses in the preventionof injuries due to sharp instruments.(Full text inChinese)
