多頻率鼓室圖的判讀與臨床應用

The Interpretation and Clinical Application of Multiple-Frequency Tympanometry

摘要


中耳功能分析中的鼓室圖測驗,分為標準鼓室圖和多頻率成分鼓室圖。鼓室圖的結果,可以讓臨床聽力師瞭解個案的中耳狀態,在聽力學診斷、聽覺管理和處遇前是相當重要的一項測試法。多頻率鼓室圖的測量藉由分析鼓室圖聲導和聲納兩成分的數值,推測中耳狀態的改變是質量或是硬度系統的異常,進而瞭解中耳病變的可能原因,對於臨床聽力學診斷有相當大的助益。本文一開始將介紹多頻率鼓室圖的原理和施測方式,同時說明如何解讀多頻率鼓室圖的結果,藉由兩名個案的聽力學檢測結果,實際說明臨床判讀的方法,同時文末將說明鼓室圖測試的限制,以及臨床上使用時應注意的事項。期待臨床工作者對未來能將多頻率鼓室圖測試,應用在臨床聽力學實務工作上,協助聽覺系統疾病的確診。

Tympanometry is a critical testing for understanding the status of middle ears and providing information for differential diagnosis. The tympanometry includes single component Y-226 Hz tympanometry and multiple-frequency tympanometry. To measure susceptance and conductance components of multiple-frequency tympanometry could provide the further information about the middle ear pathology. The single component tympanometry has been widely used in the clinical settings; however, the multiple-frequency tympanometry was not much used by clinical audiologists. The possible reasons might be the difficulty of background knowledge and the perplexity of the ways to interpret the results of the multiple-frequency tympanograms. The purposes of this article are to introduce the principles of acoustic immittance, to explain the rules of results interpretation and to emphasize the advantage and constraints of the multiplefreuency tympanometry. Two cases will be illustrated how to interpret the results and how to apply the test results for differential diagnosis.
