A Seldom Case of Nonfunctioning Pituitary Macroadenoma Incidentally Detected at [^(18)F]FDG-PET/CT

[^(18)F]FDG-PET/CT檢查偶然發現罕見非分泌性腦下垂體巨腺瘤案例

摘要


腦下垂體(pituitary gland)通常不會攝取氟-18去氧葡萄糖(2-deoxy-2-(^(18)F)fluoro-D-glucose, [^(18)F]FDG)於正子電腦斷層檢查(positron emission tomography-computed tomography, PET/CT)影像。腦下垂體攝取[^(18)F]FDG於PET/CT影像上是極為罕見。某72歲結腸癌患者因腫瘤再分期和安排治療計劃,安排兩次PET/CT,兩次PET/CT均意外發現這位患者在腦下垂體蝶鞍處強烈攝取[^(18)F]FDG。這位患者沒有出現腦下垂體腫瘤相關任何臨床的症狀。在兩次PET/CT發現有持續較高的[^(18)F]FDG攝取,最大之標準攝取值(the maximum of standard uptake value, SUV_(max))分別為4.1和4.6。我們認為,這位患者腦下垂體攝取[^(18)F]FDG於兩次PET/CT是正常生理攝取。這個意外發現(腦下垂體攝取[^(18)F]FDG)被認為是非分泌性腦下垂體巨腺瘤。

The [^(18)F]FDG (2-deoxy-2-(18F)fluoro-D-glucose) is not accumulated normally by pituitary glands on positron emission tomography-computed tomography (PET/CT) study. Focally increased pituitary [^(18)F]FDG uptake on PET/CT imaging is extremely rare. A 72-year-old female with adenocarcinoma of colon cancer was assessed with sequential PET/CT examinations for restaging and following treatment planning. The detection of an intense [^(18)F]FDG sellar region uptake in two PET/CT studies were completely unexpected. The patient did not present any clinical and laboratory sign or symptoms related to pituitary mass. Twice PET/CT studies demonstrated persistent pituitary [^(18)F]FDG higher uptake (the maximum of standard uptake value (SUV_(max)): 4.1→4.6). In our opinion, focal [^(18)F]FDG uptake on two PET/CT scans represent physiologic pituitary uptake. This case of incidental pituitary [^(18)F]FDG uptake was diagnosed as nonfunctioning pituitary macroadenoma.
